If you would like to change your Florida Medicaid MMA Plan, you may contact AHCA Choice Counseling, 1-877-711-3662, and request a new plan.  Before contacting Choice Counseling, check with your pediatrician to see what plans he/she accepts, if you wish to keep the same primary care physician for your child.
